1. On a clean sheet of paper, choose one task that you will perform (for the whole quarter) that will show your participation in community activities.
2. Copy the table below. Ask your parent/guardian to put a check if you have successfully performed the task.
3. You may also take pictures or have some documents to prove that you have done the assigned task.
4. Submit your output to your class adviser.​
